If you decide to join the panel, you will be contacted by email once or twice a month over the course of a year and be asked to provide your opinions and insights on a variety of topics.* As a participating member of the panel, you will be entered into a sweepstakes drawing to win a cash prize of $500.**
There are 4 random draws (the “Draws”) in the Sweepstakes consisting of three (3) cash prizes of
$500 each (the “Prizes”) for an aggregate of $6,000 in Prizes to be given away during the Sweepstakes
Period. The Prizes will be awarded quarterly on the last business day of the month [the “Draw Dates”], on
or around September 28, 2012, December 31, 2012, March 29, 2013 and June 28, 2013. All Prizes are to be
awarded in US dollars.
You will receive one entry for the $500 with every study you complete that you are invited to through the panel
